New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. . Any person who is over18 need to come in person to register. Proof of residence or personal ID are not a mandatory requirement and you will not be refused registration if you cannot produce them. There is  no regulatory requirement for applicants to prove identity, address and immigration status or to provide NHS number to register & it is not the role of general practice to establish identity of applicant . We do have practice policy and procedure in place, for further information please ask our registration team.


As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a new patient health check with the nurse. Please book your appointment online with our Health Care Assistant if you would like to have this Health Check done.

 Changes to Personal Details

It is very important that you keep the practice informed of any change in address / name or telephone number.  Proof of change may be requested by the practice. This is very important as it can delay medical treatment.
